A coroner's report released in November 2024 has shed light on the tragic life and death of Gilles Gaouette, a man who spent the last year of his life bouncing between the healthcare system's revolving doors.

The report, written by coroner Stéphanie Gamache, notes that Gilles Gaouette's death on December 11, 2022, was due to a lack of proper diagnosis of his dependence on benzodiazepines. This, coupled with his chronic alcoholism, led to multiple visits to the Val-d'Or Hospital's emergency room.

Gilles Gaouette's medical file, spanning nearly 1000 pages, was found to lack follow-up and centralized management, which the report states contributed to his untimely demise. The coroner's report states that Gilles Gaouette's "high use of services" was due to his practice of "revolving doors", where he would leave the emergency room once he had sobered up.

The report also highlights a significant aspect of Gilles Gaouette's case, his long-term dependence on benzodiazepines, which could have explained his confusion. The Centre Normand, a local organization specialized in addiction in Abitibi-Témiscamingue, did not focus its interventions on Gilles Gaouette's dependence on benzodiazepines, despite it seeming more severe than his chronic alcohol use disorder.

Coroner Stephanie Gamache did not blame the interveners who crossed paths with Gilles Gaouette, but identified "major structural challenges" in the healthcare system. She wrote that no one should end their life in such a way.

The report includes 18 recommendations, all aimed at improving the care of vulnerable or homeless individuals. The recommendations suggest the development of the role of a "case manager or pivot intervenor" and the establishment of "discharge plans" following an emergency room visit.

The next hearing regarding the death of Pasha Ekoomiak on February 8, 2023, is scheduled for November 17. For the death of Rayden Anichinapeo-Pien on December 29, 2022, an inquest and report will be conducted "without public hearings", according to the coroner's office. The coroner's report is the first of three announced in November 2024, following the discovery of deceased patients who had received care at the Val-d'Or hospital.

The coroner's office declined interview requests from La Presse, citing the ongoing investigations. The full report can be accessed through the Bureau du coroner du Québec or local media coverage after its release.
