The "Bergisches Land" Telenotarzt system, a joint initiative by the Ennepe-Ruhr district, Mettmann district, and the cities of Remscheid, Solingen, Wuppertal, and Leverkusen, is set to revolutionise emergency medical services by integrating telemedicine support for on-scene doctors.

The project, which began with a planning meeting at the main fire and rescue station in Leverkusen last fall, is aimed at improving emergency response efficiency and reducing doctor shortages on site. The focus in emergency services is shifting towards digitization, with this system being introduced as a supplement to existing emergency concepts.

The technical setup of the Telenotarzt system includes a built-in camera, router, antenna for video transmission, central communication control element, headsets, and other equipment. Six participating administrative bodies will have their emergency vehicles equipped with state-of-the-art technology for Telenotarzt operation, with two emergency vehicles per administrative body initially launched in a "ramp-up phase".

Pilot testing has been conducted in some areas to evaluate system usability and effectiveness in real emergency scenarios. The full operation of the Telenotarzt system is not expected for several years, but it is set to start operating in real-life conditions in spring 2025.

In an emergency, a Telenotarzt can support emergency services on-site and aid in making decisions about immediate clinical treatment. The system is largely operational in the participating regions with ongoing refinements based on feedback from emergency personnel. Some components or coverage extensions may still be in progress, with continuous updates planned to optimize functionality.

Doctors will be trained and further qualified for the Telenotarzt service, ensuring they are equipped to provide effective telemedical support. The full rollout was initially scheduled for completion during 2023 to early 2024, with phased integration in participating districts and cities. Deployment schedules might vary between localities depending on readiness and resource availability.
