In the realm of modern medicine, the integration of Artificial Intelligence (AI) holds immense potential for improving outcomes, enhancing efficiency, and offering more personalized, compassionate care. However, a significant trust gap exists between patients and clinicians regarding the use of AI in healthcare systems.

Multiple studies have shown that patients tend to trust physicians less when AI is explicitly mentioned in their care, fearing depersonalization, over-reliance on technology, and potential loss of human connection [1][3]. On the other hand, healthcare professionals generally view AI as a tool to improve outcomes and efficiency, displaying greater optimism than patients [5].

To bridge these trust gaps, it is crucial to:

  1. Ensure human oversight and involvement: AI should augment, not replace, clinicians’ decision-making. Patients trust increases when AI outputs are combined with clinicians' expertise and communication, preserving the personalized human interaction [3][5].
  2. Preserve empathy and relational care: AI systems must be designed to support empathetic communication and attentive care, avoiding over-automation that risks depersonalizing care or making patients feel "ghosted."
  3. Build transparency and clear communication: Provide patient-centered explanations about how AI is used in their care, enhancing understanding and trust.
  4. Address ethical, privacy, and bias concerns: Demonstrate that AI systems comply with data privacy, ethical standards, and actively mitigate biases to assure equitable care, which fosters trust from diverse patient populations [2].
  5. Integrate AI thoughtfully into existing workflows: AI solutions must align with healthcare infrastructure and clinicians' work patterns, supporting rather than disrupting practice or increasing medicolegal risks [2][3].

Recent findings from the 10th annual Future Health Index (FHI) report by Royal Philips, a global health technology leader, indicate that AI has the potential to double patient capacity by 2030, as AI agents assist, learn, and adapt alongside clinicians [6]. However, concerns remain about the implementation of AI, with over 75% of respondents being unclear about liability for AI-driven errors [7].

In many countries, patients face significant wait times for specialist appointments. More than half of the 16 countries surveyed revealed patients are waiting nearly two months or more for specialist appointments [8]. Furthermore, the FHI 2025 report reveals 33% of patients have experienced worsening health due to delays in seeing a doctor, with more than 1 in 4 ending up in the hospital due to long wait times [9].

The trust gap between patients and AI in healthcare is a crucial issue that must be addressed to fully realise the potential benefits of AI in patient care. By focusing on human-centered design, collaboration with clinicians, and transparency, we can narrow the clinician-patient trust gaps, improve patient perceptions of AI-driven care, and unlock AI’s full potential to enhance outcomes, efficiency, and patient satisfaction.
